For new, current and reactivating patients: below are some of the forms necessary to create/update your patient records.

Client Intake Packet – to be completed by all new and reactivating patients.

Medical Initial History Form | Medical History Update Form – to be completed for any medical changes/updates, as well as before your yearly physical exam.

Specialty Care Clinic staff will not share your patient information or confirm any individual is in our care without a signed and dated authorization form from you before the request is made.
