The Northeast Health District’s Child Health program is seeking a qualified person to provide home visits, assessments, referrals, and coordination of services for eligible clients through the Children 1st program. The specialist will conduct developmental screenings and related assessments under the direction of the Early Childhood Program Coordinator. This is a full-time position that will require frequent travel within the district.

Job Responsibilities & Performance Standards:   

  1. Provides home visits, phone calls, and other forms of care coordination for eligible clients.
  2. Provides developmental screening, assessment, and intake of clients referred to Child Health programs.
  3. Plans and coordinates delivery of intervention activities, including referrals.
  4. Maintain knowledge of Child Health policies, procedures, and scope of services.
  5. Collaborates with Georgia Division of Family & Children Services (DFCS) and other community partners.
  6. Maintains consistent communication and coordinates services and activities within Children 1st Program and throughout the district.
  7. Meets with multi-disciplinary team on a scheduled basis.
  8. Completes data entry for recordkeeping and documentation.
  9. Participates in quality management meetings to collaborate, identify, and report on issues such as barriers to care.

Minimum Qualifications:
HCP120-$40,000
Master’s degree in a related field from an accredited college or university AND eligibility to be licensed.
OR
Bachelor’s degree in a related field from an accredited college or university AND Two years of experience in social service delivery

HCP121-$43,000
Master’s degree in a related field from an accredited college or university AND One year of experience AND eligibility to be licensed.
OR
Bachelor’s degree in a related field from an accredited college or university AND Three years of experience in social service delivery
OR
One year of experience at the lower-level Social Worker, NonLicensed 1 (HCP120) or position equivalent
